Employees & Non-employee Collaborators (Students/Interns/Researchers) ending their terms with the CGM should complete a CGM EMPLOYEE DEPARTURE FORM

Non-Visa holders should submit a Departure Form no later than 2 weeks prior to their departure from the CGM.

Visa Holders MUST submit a Departure Form 30 days prior to their last day, as visa termination requests must be submitted to the GPS International Office.

If applicable, please bring any keys or borrowed items (Partners/MGB hospital property) to Jim Mull in Simches 5 Admin Offices.

If you’re on site, you can interoffice mail or bring your ID badge to the Police & Security located on the MGH Main Campus, in the Wang Building, Floor 2, suite 232.

If you’re working from home please mail your MGH badge via regular mail to the below address:

CGM Employee Departure Form

Network Extension Request (NER)

In some cases, an employee leaving the CGM may need access to their emails for a period after their official last day.

Network Extension Requests (NERs) are processed through the IS Service Hub. If approved, this will grant the employee departing the MGH, with access to either their H-Drive, Outlook Email, or both.

To ensure no interruption in access, NERs should be submitted at least 3-4 weeks before the departing employee’s last day by a manager/supervisor. However, if that is not possible IS will allow a request to be submitted no later than 1-week after the departure date.

Forwarding Email to an External Email Account.

Oftentimes, employees who are leaving their jobs call the Service Desk to request that any emails sent to their Partners email accounts, after their last day of work, be automatically forwarded to their private email accounts.

PHS email systems does not allow the forwarding of email due to PHS privacy concerns and/or HIPAA constraints. The requestor should plan accordingly, as all email will be deleted when their account terminates with their employment.
